• Payor Name
• Payor Address (address 1, address 2, City, State, ZIP)
• Parent Who Owes Support (Address 1, Address 2, City, State, ZIP, Country)
• Parent Who Owes Support Social Security Number
• Parent Who Owes Support Case Number *

*If the Parent Who Owes Support has multiple case numbers, only one is needed to register
